My Dodge Ram starts rough any ideas?

I have a 1999 Dodge Ram 1500 with the 5.9L engine. When it's cold and I start it the RPM jumps like a normal car to about 2k and then dies if I don't give it the gas and keep it around 1k RPM for a few seconds. Sometimes it will shutter for a little while too. So I cleaned the throttle body really well, put in a new air filter and reset the ecu. After that it started fine for a little while then the problems started again. I have a friend with a 1990 chevy that is doing the same thing. Any ideas? I'm lost
