I wrote this poem and ...?

There are no metaphors in your poem tho it is good. A simile is when you use a sentence with the words "like" or "as" in eg - She was like a swan when swimming in the water. A metaphor is when you describe something by making it something else eg- She was a swan in the water. Therefore, maybe you could add a metaphor into verse two "Illness is not my choice," could maybe become "illness surrounds me, drowning me" or a simile "illness surrounds me like a rock concert in stereo!"
